- Where you do unfair, unsafe, or dangerous work or stay in a dangerous situation? If you don’t work or try to leave, someone might hurt you or someone you love? Yes or No
- Where you are not able to keep or hold onto your identification or travel documents? Yes or No
- Where you are not able to contact your family, spend time with your friends, or come and go as you please? Yes or No
- Where you are not able to live where you want and live where you work or told by those in charge? Yes or No
- Where someone told you to lie about the kind of work you’re doing or will be doing? Yes or No
- Where your family or loved ones are hurt and/or threatened by your employer if you do not work or try to leave? Yes or No
- Where you traded sex for money, drugs, a place to stay, a cell phone, safety or something else? Yes or No
- Where you are asked or forced to do something sexual with another person by your significant other or employer without your consent? Yes or No
